insert new row into a database (Data sources/Odata services)

Hi,

I have a database set up and imported into SAP Appbuilder web-browser client.

What i am trying to do now, is to try to add in value/text into the database.

The database has a Unique ID which is auto-generated and 2 more columns, Year and School which i need to update rows into.

This is the Ajax to insert new row into the existing database call SurveyDB

The GUI im using right now where user will select the options are as Below

There is a button at the bottom in which user are able to click to submit/update to the database with their selection.

Any help will be much appreciated.
