I heard back from SAP today on my incident about the missing last name. They told me to apply note 2291518 and they also noticed that I had configured the full contact name for my ALE under the first field. According to the User guide, this is wrong and I should have configured it differently (see section 2.5 of the user guide for details). We had already applied note 2291518 on Monday, so I just made the suggested configuration change and that fixed the issue with the missing last name.
SAP also advised me that they are still researching the wrong phone number showing in the XML (showing phone number from 1095, when it should show phone number from 1094). I guess this is only applicable to companies who configured two different numbers like we did.
